Urgent Community Response: @home
The urgent response component of @home is for people who:
- are 18 or over and a resident of Southwark
- are medically stable / predictable
- live in the community and require an urgent response within 0-2 hours, or same/next day to:
- prevent an avoidable admission to hospital
- prevent a clinical deterioration
- prevent a social crisis
- prevent admission to A&E
- prevent admission to emergency residential or nursing care
Opening times for the Urgent Community Response @home service are the following:
- referrals can be made between 8.30am to 6pm, 7 days a week
- care and support can be provided between 8.30am to 9.30pm, 7 days a week
