Southwark Healthy Lifestyle hub

This free service provides support to residents who would like to make positive changes, but need a little bit of help to get going.

They provide friendly support to motivate you, as well as access to a range of services.

If you need a bit of help with motivation, call the Healthy Lifestyle Hub and they can help you to become and stay physically active.

This service is for Southwark residents only, or those registered with a Southwark GP. You need to be over the age of 18 and have one or more of the following: 

  • a low level of physical activity
  • smoking
  • obesity
  • struggling with low mood, stress or anxiety

Family physical activity and healthy eating course - Alive ‘N’ Kicking

Alive ‘N’ Kicking is a free 12 week course to help whole families make healthier choices around food and exercise. It helps you make small, easy changes to lose weight and benefit their health, through increasing physical activity and eating more healthily.

Alive ‘N’ Kicking is for children (4 to 17 years) and their families that live in Southwark, who are overweight or obese. If you think this is you, give the team a call on 0333 005 0095.

Exercise on referral

Exercise on referral is a 12 week programme that can support you to increase the amount of physical activity you do each week.

Cardiac (heart) rehabilitation services in Southwark

If you have had a heart or circulation problem and been treated in hospital - for example, a heart attack or heart surgery - you should be invited to a cardiac rehabilitation service. This service includes exercise, education and relaxation, led by the hospital rehabilitation team. The service aims to help you:

  • manage your condition
  • regain your quality of life
  • increase your physical activity and ability to exercise

Pulmonary (lung) rehabilitation services in Southwark

Pulmonary rehabilitation will help you if you're living with long-term (chronic) lung disease, such as:

  • chronic obstructive pulmonary disease (COPD)
  • asthma
  • bronchiectasis

Pulmonary rehabilitation may also help if you've had COVID-19 and are:

  • feeling breathless
  • your chest feels tight
  • you're feeling tired a lot or all of the time

Pulmonary rehabilitation is a course of education and physical activity that helps you to learn more about your condition. It will improve your fitness and strength, and feel less breathless. By understanding your condition and knowing how to cope with it, you can feel back in control again. It works very well and can make a real difference to your life.

Diabetes prevention programme

The Healthier You Diabetes Prevention Programme provides personalised support to help adults reduce their risk of type 2 diabetes. This includes education on healthy eating and lifestyle, help to lose weight, and physical exercise programmes. Together, these have been proven to reduce the risk of developing this condition. 

If a blood test shows that you're at risk of type 2 diabetes, you should be referred to the service. You can self-refer to the diabetes prevention programme or ask your GP for more information.

If you've had a fall, or are afraid of falling, ask your GP or another health professional to refer you to the falls prevention service.
