How to apply
Please note - You must apply to the licensing authority of the district or borough in which you live, not where you work. If you live outside Southwark, apply to your local authority.
You must be 18 years or older and hold a licensing qualification such as the Award for Personal Licence Holders (APLH) Level 2.
Before you apply you must have:
- a standard DBS check – this must be less than one month old when you submit your personal licence
- an accredited qualification
- proof of the right to work in the UK

If a third party (agency, solicitor etc) has applied for the DBS check on the applicant's behalf and the DBS check doesn't show the residential address once issued, we ask you to forward us proof of address (bank statement, utility bill, Council Tax bill, driving licence) to confirm you live within the Southwark borough.
Information you need to include
Your application must include:
- 2 passport sized photographs with a white background, one of the photographs must be signed by an approved professional
- proof of right to work in the UK – for example, a current passport or birth certificate
- any licensing qualification that proves you can sell alcohol
- DBS disclosure form.
- a completed certificate of disclosure of convictions form
- payment of the fee for the application
